A regeneration-based, nonobturation root-canal treatment for fully-mature teeth: Six years' experience with "SealBio" |

Naseem Shah DOI:10.4103/0976-237X.188541 PMID:27630490Objectives: To provide scientific evidence on the outcome of a large number of cases treated by SealBio over the longer follow-up period. Materials and Methods: One hundred and thirty-four teeth in 116 patients presenting with pulp and periapical disease were randomly recruited between 2009 and 2014. SealBio was performed, and cases were followed up at regular intervals up to 6-year. Results: Of the total 134 teeth treated, 16 teeth could not be followed up and 9 cases failed (7.62% of cases). In only 4 cases (approximately 3.38% of cases), the failure could be directly attributed to endodontic causes. In the remaining 5 cases, coronal leakage from under the crown margins or dislodged restoration was found after 3-5 years of treatment. Conclusions: SealBio was found to be a successful, nonobturation, regeneration-based endodontic treatment protocol. By cell homing of endogenous stem cells, a biological seal rather than an artificial seal with gutta-percha and sealer cement is possible to achieve. It is highly cost saving and easier to perform, in addition to other advantages, such as retreatment is much simpler, and postcore restoration is possible after SealBio treatment. |

Evaluation of chemokines in gingival crevicular fluid in children with band and loop space maintainers: A clinico-biochemical study |

Aim: This study was conducted to detect the presence of chemokines, macrophage inflammatory protein-1α (MIP-1α), and 1β (MIP-1β) and estimate their levels in gingival crevicular fluid (GCF) in children with band and loop space maintainers. Materials and Methods: MIP-1α and MIP-1β levels were estimated in GCF samples from twenty healthy children and twenty children with band and loop space maintainers. Periodontal status was evaluated by measuring gingival index, plaque index, and Russell's periodontal index. The GCF samples were quantified by ELISA, and the levels of MIP-1α and MIP-1β were determined. Results: The mean MIP-1α concentrations in healthy children and those with space maintainers were 395.75 pg/µl and 857.85 pg/µl, respectively, and MIP-1β was 342.55 pg/µl and 685.25 pg/µl, respectively. MIP-1α and MIP-1β levels in GCF from children with space maintainers were significantly higher than in the healthy group, and statistically significant difference existed between these two groups. Conclusion: MIP-1α and MIP-1β can be considered as novel biomarkers in the biological mechanism underlying the pathogenesis of gingival inflammation in children with space maintainers. |

Effect of nanoscale particles incorporation on microhardness of polymers for oral prosthesis |

Materials and Methods: Twenty-one discs of each resin were fabricated, whereas seven had no additive, seven had 3% of nanoscale pigments and last seven had 10% of them. The percentage was obtained by measuring the total weight of each resin disc. Besides, seven discs composed by only nanoscale pigments were also fabricated, totalizing 91 discs. The pigment was weighed by using an analytical balance (BEL Analytical Equipment, SP, Brazil). The surface hardness was measured through a hardness tester machine before and after thermocycling (5-55°C, for 2000 cycles). Data were analyzed by ANOVA and Tukey's test (P < 0.05). The chemical composition of the discs composed only by nanoscale pigments was analyzed with EDS test. Results: Hardness of all resins decreased after thermocycling. The lowest values were observed on the discs with 3% of nanoscale pigments and discs fabricated only with them. EDS showed the presence of titanium dioxide. Conclusion: Discs with 7% of pigments (after thermocycling) showed higher hardness values. |

Impact of hydrogen peroxide activated by lighting-emitting diode/laser system on enamel color and microhardness: An in situ design |

This in situ study evaluated the influence of 15% and 35% HP gel activated by lighting-emitting diode (LED)/laser light for in-office tooth bleaching. Materials and Methods: Forty-four bovine enamel slabs were polished and subjected to surface microhardness (load of 25 g for 5 s). The specimens were placed in intraoral palatal devices of 11 volunteers (n = 11). Sample was randomly distributed into four groups according to the bleaching protocol: 15% HP, 15% HP activated by LED/laser, 35% HP, and 35% HP activated by LED/laser. The experimental phase comprised 15 days and bleaching protocols were performed on the 2 nd and 9 th days. Surface microhardness (KHN) and color changes were measured and data were analyzed by ANOVA (α = 0.05). Results: There were no significant differences in microhardness values neither in color alteration of enamel treated with 15% HP and 35% HP activated or not by LED/laser system (P > 0.05). Conclusions: Both concentrations of HP (15 or 35%), regardless of activated by an LED/laser light, did not affect the surface microhardness and had the same effectiveness in enamel bleaching.

Plaque removal efficacy of Colgate 360 toothbrush: A clinical study |

Study Design: This was a single-center, monadic, case-controlled study with the 7 days duration. Materials and Methods: A total of eighty participants (56 male and 24 female) aged between 18 and 45 years with a minimum of 20 permanent teeth (excluding the third molars) without any prosthetic crowns and an initial plaque score of minimum 1.5 as determined by Modified Quigley-Hein Plaque Index (1970) participated in the study. There were two dropouts during the study duration, one male and one female. The participants were instructed to brush for 1 min, after which plaque index was recorded again. They were then instructed to brush their teeth twice a day for 1 min with the assigned toothbrush (Colgate 360 Whole Mouth Clean Toothbrush) and a commercially available fluoride toothpaste for the next 7 days. On the 7 th day, all the participants were recalled for follow-up and plaque examination. The plaque index scores (pre- and post-brushing) were recorded, tabulated, and analyzed statistically. Results: The mean plaque indices reduced after brushing both on day 1 and day 7. There was also a reduction in mean plaque indices from day 1 to day 7. All these reductions were statistically significant (P < 0.001). The reduction in plaque scores was independent of the gender of the participants however female participants showed lower scores as compared to male participants (P < 0.001). Conclusion: The present study demonstrated a significant reduction in plaque scores with the use of Colgate 360 Whole Mouth Clean Soft Toothbrush throughout the study period. Continued use resulted in a further significant reduction in plaque scores irrespective of the gender of participants. |

Evaluation of sealing ability of Biodentine™ and mineral trioxide aggregate in primary molars using scanning electron microscope: A randomized controlled in vitro trial |

Study Design: The study sample comprised forty recently extracted primary molars. These teeth were placed in a 5.25% sodium hypochlorite solution for 24 h and washed with tap water. Access cavities were made using a round bur in high-speed handpiece. Perforations were made in the center of the floor of the pulpal chamber using a 0.5 mm round bur. The teeth were randomly assigned into two experimental groups based on the material used to seal the perforation: Group A - MTA and Group B - Biodentine™. The packed materials were allowed to set for 24 h. The samples were sectioned longitudinally and the extent of marginal adaptation was measured by SEM. Wilcoxon-signed rank test was used for statistical analysis using SPSS software. Results: All teeth exhibited microleakage, but Biodentine™ showed significantly less leakage (0.149) compared to MTA (0.583). Conclusion: Based on the results of this study, Biodentine™ showed lesser microleakage compared to MTA and thus may be a good alternative to MTA. |

Comparative evaluation of shear bond strength and nanoleakage of conventional and self-adhering flowable composites to primary teeth dentin |

Few studies have addressed resin bonding to primary teeth. Aim: The aim of this study was to compare the shear bond strength and nanoleakage of conventional and self adhering flowable composites to primary teeth dentin. Settings and Design: This study was conducted in the Department of Pedodontics and Preventive Dentistry, I.T.S Dental College, Hospital and Research Centre, Greater Noida; in association with the Department of Mechanical Engineering, I.T.S Engineering College, Greater Noida; and the Advanced Instrumentation Research Facility (AIRF), Jawaharlal Nehru University, New Delhi. Materials and Methods: Sixty of the ninety primary teeth were evaluated for shear bond strength and thirty for nanoleakage. The samples were divided into three groups; Group I - Dyad Flow (Kerr), Group II - Fusio Liquid Dentin (Pentron Clinical Technologies) and Group III - G-aenial Universal Flo (GC). Shear bond strength was determined using a universal testing machine. Nanoleakage pattern was observed under scanning electron microscope. Results: The shear bond strength of conventional flowable composite was significantly greater than self adhering flowable composite (p<0.05). Nanoleakage scores of both conventional and self adhering flowable composites were comparable. Conclusions: Self adhering flowable composites combine properties of composites and self etch adhesives, eliminating the need for separate bond application that simplifies direct restorative procedure. The evolution of self adhering materials could open new horizons for pediatric dentistry. |

Evaluation of pulpal response of deciduous teeth after direct pulp capping with bioactive glass and mineral trioxide aggregate |

Roza Haghgoo, Motahare Ahmadvand DOI:10.4103/0976-237X.188552 PMID:27630497Aim: The aim of this study was to evaluate the pulpal response of primary teeth after direct pulp capping (DPC) with two biocompatible materials namely mineral trioxide aggregate (MTA) and bioactive glass (BAG). Settings and Design: This study was a randomized clinical trial. Materials and Methods: A total of 22 healthy primary canine teeth scheduled for extraction for orthodontic reasons were selected. The teeth were divided into two groups of 11 and underwent DPC. The exposure sites were randomly capped with MTA or BAG in the two groups. After 2 months, the teeth were extracted and prepared for histopathologic evaluation. Statistical Analysis: The data were analyzed using Fisher's exact test. Results: In the BAG group, inflammation was seen in three patients; internal resorption and abscess were not seen at all. In the MTA group, inflammation was seen in one patient and internal resorption and abscess were not seen in any patient. Fisher's exact test showed no significant difference between the two groups (P > 0.05). Dentinal bridge formation was noted in five patients in the BAG group and six patients in the MTA group. No significant difference was observed between the BAG and MTA groups using Chi-square analysis (P = 0.67). Conclusion: Based on the results of this study, MTA and BAG can be used for DPC of primary teeth. |

Comparative assessment of the effectiveness of different cleaning methods on the growth of Candida albicans over acrylic surface |

Aims: To find the best possible method for maintaining denture hygiene. Settings and Design: This retrospective analysis was conducted in the Guru Nanak Institute of Dental Sciences and Research, Kolkata, and this in vitro study was designed to minimize denture stomatitis among denture wearing population. Subjects and Methods: Sixty acrylic discs of equal dimensions after exposure to C. albicans were treated for a duration of 24 h with denture adhesive, cleanser, 0.2% chlorhexidine individually, or in combinations simulating clinical conditions dividing in six groups, ten samples each (n = 10). Statistical Analysis Used: After treatment, colony count was evaluated and statistically analyzed by post hoc Tukey's test and Dunnett's test to determine the most effective way of prevention. Results: The statistical post hoc analysis (Tukey's test and Dunnett's test) showed high significance (P < 0.0001). The group treated with adhesive showed high fungal growth compared to the control group, whereas chlorhexidine showed high potency to prevent C. albicans, whereas adhesive increased the adhesion of C. albicans to acrylic surface. Conclusions: Denture adhesive increases the adherence of C. albicans to denture surface. Other cleaning chemicals such as cleanser and chlorhexidine decrease the adherence. Moreover, among the all denture cleaning protocol, chlorhexidine drastically inhibit the adherence, as well as growth of C. albicans over denture surface. |

Expression of periodontal inflammation into left ventricular hypertrophy in Type 2 diabetes mellitus: A cross-sectional study |

These conditions, occurring as comorbidities, synergically affect periodontal tissues. Aim: This study aims to examine whether chronic gingivitis and chronic generalized severe periodontitis in patients with Type 2 diabetes mellitus are associated with increased left ventricular mass (LVM). Materials and Methods: A total of 45 patients affected with Type 2 diabetes mellitus were recruited and divided into three groups with 15 patients each according to their periodontal status: Group I consisting of healthy individuals, Group II consisting of chronic gingivitis, and Group III consisting of chronic generalized severe periodontitis. They were assessed clinically, biochemically, and echocardiographically. LVM was calculated according to Devereux formula and was indexed to height. Results: The differences in the means for LVM and LVM index (LVMI) were statistically significant in three groups with a P = 0.006 and 0.014, respectively. After adjusting for the confounders, the mean values of LVM in Group I, II, and III were 149.35 ± 35.51 g, 147.95 ± 31.59 g, and 156.36 ± 36.57 g, respectively and for LVMI, the mean values were 43.61 ± 12.16 g/m 2.7 (Group I), 47.12 ± 10.84 g/m 2.7 (Group II), and 46.34 ± 12.55 g/m 2.7 (Group III). Conclusions: A positive association between chronic generalized severe periodontitis and increased LVM in Type 2 DM patients was observed, suggesting the role of periodontal disease in the left ventricular hypertrophy. |

Interaction of IL1B and IL1RN polymorphisms, smoking habit, gender, and ethnicity with aggressive and chronic periodontitis susceptibility |

Aims: To investigate the importance of IL1B-511C>T (rs16944), IL1B +3954C>T (rs1143634), and IL1RN intron 2 variable number tandem repeat (VNTR) (rs2234663) polymorphisms, individually or in combination, as the risk factors of periodontitis in a Southeastern Brazilian population with a high degree of miscegenation. Subjects and Methods: A total of 145 individuals, with aggressive (aggressive periodontitis [AgP], n = 43) and chronic (chronic periodontitis [CP], n = 52) periodontitis, and controls (n = 50) were genotyped by polymerase chain reaction (PCR) (IL1RN intron 2 VNTR) or PCR-restriction fragment length polymorphism (PCR-RFLP) (IL1B-511 C>T and IL1B + 3954C>T) techniques. Statistical Analysis: The independent t-test, Chi-square, and Fisher's exact tests were used. The SNPStats program was used for haplotype estimation and multiplicative interaction analyses. Results: The IL1B +3954T allele represented risk for CP (odds ratio [OR] = 2.84), particularly in smokers (OR = 4.43) and females (OR = 6.00). The minor alleles IL1RN*2 and *3 increased the risk of AgP (OR = 2.18), especially the IL1RN*2*2 genotype among white Brazilians (OR = 7.80). Individuals with the combinations of the IL1B + 3954T and IL1RN*2 or *3-containing genotypes were at increased risk of developing CP (OR = 4.50). Considering the three polymorphisms (rs16944, rs1143634, and rs2234663), the haplotypes TC2 and CT1 represented risk for AgP (OR = 3.41) and CP (OR = 6.39), respectively. Conclusions: Our data suggest that the IL1B +3954C>T and IL1RN intron 2 VNTR polymorphisms are potential candidates for genetic biomarkers of periodontitis, particularly in specific groups of individuals. |

Evaluation of moxifloxacin-hydroxyapatite composite graft in the regeneration of intrabony defects: A clinical, radiographic, and microbiological study |

An anti-microbial agent, i.e. moxifloxacin has been incorporated into the bone graft to control infection and facilitate healing during and after periodontal therapy. Materials and Methods: By purposive sampling, 15 patients with at least two contralateral vertical defect sites were selected. The selected sites in each individual were divided randomly into test and control sites according to split-mouth design. Test site received moxifloxacin-hydroxyapatite composite graft and control site received hydroxyapatite-placebo gel composite graft. Probing depth (PD) and Clinical attachment level (CAL) were assessed at baseline, 3, 6, 9, and 12 months. Bone probing depth (BPD) and hard tissue parameters such as amount of defect fill, percentage of defect fill, and changes in alveolar crest were assessed at baseline, 6, and 12 months. Changes in subgingival microflora were also assessed by culturing the subgingival plaque samples at baseline and at 3-month follow-up. The clinical, radiographic, and microbiological data obtained were subjected to statistical analysis using descriptive statistics, paired sample t-test, independent t-test, and contingency test. Results: On intragroup comparison at test and control sites, there was a significant improvement in all clinical and radiographic parameters. However, on intergroup comparison of the same, there was no statistically significant difference between test and control sites at any interval. Although test sites showed slightly higher amount of bone fill, it was not statistically significant. There was a significant reduction in the counts of Aggregatibacter actinomycetemcomitans and Porphyromonas gingivalis at both sites from baseline to 3 months. In addition, there was a significant reduction at test sites as compared to control sites at 3-month follow-up (P = 0.003 and P = 0.013). Conclusion: The reduction in microbial counts found in test sites at 3-month follow-up could not bring similar significant improvements in the clinical and radiographic parameters though the test sites showed slightly higher bone fill. |

Materials and Methods: For evaluation of various facial proportions, standardized frontal facial photographs of total 300 subjects between 18 and 30 years of age were obtained. Black and white copies of these photographs were presented in front of an evaluation jury for assigning scores of facial attractiveness and finally 130 attractive subjects were selected. These subjects were divided into two groups, Group I (attractive females n = 65) and Group II (attractive males n = 65) and they were further analyzed for various parameters of facial proportions. Unpaired Student's t-test was used to compare both groups. Results: Group I showed that five of seven vertical facial proportions were close to divine proportion (1.618) whereas only two vertical proportions in Group II were close to it. Transverse facial proportions in both groups deviated more from divine proportion (1.618) and were closer to silver proportion (1.414). Conclusions: Most of the facial proportions of attractive females in the North-Indian population were close to the divine proportion. Thus, facial divine proportion could be an important factor in the perception of facial attractiveness of North-Indian attractive females. |

A comparative evaluation of extraction socket preservation with demineralized freeze-dried bone allograft alone and along with platelet-rich fibrin: A clinical and radiographic study |

Materials and Methods: A randomized controlled clinical trial was carried out on 36 nonrestorable single-rooted teeth sites. Sites were randomized into demineralized freeze-dried bone allograft (DFDBA) combined with PRF - test and DFDBA - control groups using a coin toss method. After the placement of graft material, collagen membrane was used to cover it. The clinical parameters recorded were ridge width and ridge height. All the parameters were recorded at baseline and at 90 and 180 days. Statistical Analysis Used: Independent t-test and paired t-test. Results: In both groups, there is significant reduction in loss of ridge width and ridge height from baseline to 90 days (P < 0.001), baseline to 180 days (P < 0.001), and 90-180 days (P < 0.001). However, when both the groups were compared the test group favored in the reduction of ridge width while there was no statistical difference in reduction of ridge height among at different intervals. Conclusions: Although DFDBA is considered as an ideal graft material, PRF can be used as an adjunctive with DFDBA for socket preservation. |

Comparative evaluation of co-enzyme Q10 and Melaleuca alternifolia as antioxidant gels in treatment of chronic periodontitis: A clinical study |

Coenzyme Q10 and tea tree oil (TTO) are known to have potential therapeutic benefits in chronic periodontitis. Aims: The aim of the study is to compare the efficacy of Coenzyme Q10 (Perio Q® ) and tea tree oil (Melaleuca alternifolia) gel as an adjunct to scaling and root planing in the treatment of chronic periodontitis. Materials and Methods: Patients were divided equally into three groups: Group I (Control group): those receiving placebo gel + SRP, Group II (Test group I): those receiving Perio Q TM gel + SRP, and Group III (Test group II): those receiving tea tree oil gel + SRP. A total of 15 patients with 45 sites were enrolled in the study. Clinical parameters evaluated were plaque index (PI), gingival bleeding index (GI), probing pocket depth (PPD), and clinical attachment level (CAL). Statistical Analysis Used: Paired t-test was applied using SPSS software. Results: Mean PPD reduction for Group I, Group II, and Group III was 0.50 ± 0.2, 2.95 ± 0.20, and 2.09 ± 0.15, respectively. Mean CAL reduction for Group I, Group II, and Group III was 0.45 ± 0.22, 2.33 ± 0.04, and 2.28 ± 0.09, respectively. Changes in mean PI scores for Group I, Group II, and Group III were 0.67 ± 017, 1.00 ± 0.11, and 1.08 ± 0.05 and GBI scores were 0.92 ± 0.29, 1.08 ± 0.13, and 0.88 ± 0.28, respectively. Conclusions: Coenzyme Q10 and tea tree oil gel proved to be effective in the treatment of chronic periodontitis. |

Correction of Class II malocclusion and soft tissue profile in an adult patient |

Class II malocclusion with bialveolar protrusion often dictates premolar extractions with maximum anchorage. The present article describes the case of an adult female with skeletal Class II malocclusion, bimaxillary protrusion, increased overjet, deep bite, lip protrusion, everted lower lip, deep mentolabial sulcus, and lip incompetence. To correct the malocclusion, all four first premolars were extracted. Direct anchorage from miniscrews was used for retraction of the anterior segment. The mandibular buccal segment was protracted into the extraction space using Class II mechanics. Ideal Class I canine and molar relation were achieved in 24 months. There was a significant improvement in facial profile and smile esthetics of the patient. |

Orthodontic-orthognathic interventions in orthognathic surgical cases: "Paper surgery" and "model surgery" concepts in surgical orthodontics |

Presurgical planning (paper surgery and model surgery) are the most essential prerequisites of orthognathic surgery, and orthodontist is the one who carries out this procedure by evaluating diagnostic aids such as crucial clinical findings and radiographic assessments. However, literature pertaining to step-by-step orthognathic surgical guidelines is limited. Hence, this article makes an attempt to provide an insight and nuances involved in the planning and execution. The diagnostic information revealed from clinical findings and radiographic assessments is integrated in the "paper surgery" to establish "surgical-plan." Furthermore, the "paper surgery" is emulated in "model surgery" such that surgical bite-wafers are created, which aid surgeon to preview the final outcome and make surgical movements that are deemed essential for the desired skeletal and dental outcomes. Skeletal complexities are corrected by performing "paper surgery" and an occlusion is set up during "model surgery" for the fabrication of surgical bite-wafers. Further, orthodontics is carried out for the proper settling and finishing of occlusion. Article describes the nuances involved in the treatment of Class III skeletal deformity individuals treated with orthognathic surgical approach and illustrates orthodontic-orthognathic step-by-step procedures from "treatment planning" to "execution" for successful management of aforementioned dentofacial deformity. |

In addition, prototyped tridimensional models allow the surgical procedure to be simulated and enable important anatomic structures to be visualized. To present a bone defect rehabilitated with xenogenic bone preshaped on a stereolithographic model and the follow-up after 7 years of treatment. The present case report describes a bone defect rehabilitated with a lyophilized bone block preshaped on a stereolithographic model. The patient, a 56-year-old woman, was referred to the dental office presenting a bone defect in the anterior maxilla. Bone regeneration intervention was performed with xenogenic grafting and barrier membrane. The follow-up of the postoperative period and after 7 years is presented. After 7 years, the tomographic exam showed the maintenance of bone at the grafted site, representing the long-term success of the treatment. |

Neuroblastoma in early childhood: A rare case report and review of literature |

It can be sporadic or nonfamilial in origin. It is primarily a tumor of abdominal origin from where it metastasis to lymph nodes, liver, intracranial and orbital sites, and central nervous system. There is no standard dental treatment protocol for the management of neuroblastoma due to its poor survival rate and rarity. However, dental treatment may follow the protocol of preventive and restorative. Surgicals should be performed under supervision as it may trigger metastasis. We report a rare case of neuroblastoma in a 3-year-old child presenting classical oral manifestations such as bilateral palatal swelling, rolled border ulcer on the posterior part of hard palate adjacent to primary molars, and bilateral proptosis. |

Management of a massive compound odontoma in a 9-year child |

The denticles were removed after the removal of retained 74 completely and wound healing was observed. Odontomas are considered as hamartomatous dental malformation rather than true neoplasm of odontogenic origin. They are the most commonly occurring abnormally formed dental tissues that interfere with eruption of associated teeth. The eruption disturbances seen due to odontomas are delayed eruption or deflection of associated teeth. These malformations are usually asymptomatic and discovered during routine radiographic investigations. Correct diagnosis followed by proper treatment plan results in a favorable prognosis. |

Simultaneous correction of functional posterior cross bite and midline shift |

This posterior crossbite is associated with anterior crowding which is presented as an infrapositioned canine or a palatally positioned lateral incisor on one side; this leads to an upper midline shift toward the crowded side. The present case report shows the management of posterior crossbite with functional shift and upper midline shift simultaneously without adverse side effects. In this case, rapid maxillary expansion along with fixed appliance is used to correct posterior crossbite with the upper dental midline shift using reciprocal action of elastic transseptal fibers. |
